An adnexal ovarian mass including ovarian endometriosis is one of the most common (1-3%) complications during pregnancy. Frank neoplastic ovarian tumors are surgically removed early in the second trimester to avoid acute abdomen during pregnancy. On the other hand, ovarian endometriosis is usually managed conservatively during pregnancy.

Until now, there has been scant evidence for the efficacy of this wait-and-see strategy. Further, effects of pregnancy upon ovarian endometriosis have not been well studied; however, pseudo-pregnancy therapy, including low-dose contraception pills, has been successfully used to treat endometriosis.
